Review product documents and procedures
Before handling a material, review its product label, lot documentation and your organisation's approved procedure. A general web guide cannot determine the solvent, volume, concentration, container or stability period.
Record each reconstitution
Complete the record as the work is performed, using the fields in the template. Do not change or interpret a result after the fact.
Label the resulting container
Use your organisation’s approved labelling system and record the original vial and lot documentation associated with the new container. Refer any departure from the procedure for review through your quality process.
Reconstitution records
Use this template to document reconstitution performed by trained, authorised personnel. It does not specify volumes, concentrations, administration or clinical instructions.
